Schedule Your Appointment & Complete Online Patient Forms
We offer two convenient ways to get scheduled: you can call us directly, or you can take care of everything online. We’re here 5 days a week and are open until 7 PM on Mondays and Thursdays, so it should be easy to find a time that works perfectly for you. Before your visit, please also complete our online New Patient Forms.
Step 3:
Your Initial
Dental Exam
Using digital X-rays along with a visual and tactile exam, Dr. Julovich and our team will give you a very detailed checkup that looks at your teeth, gums, bite, and any pre-existing dental work. We’ll frequently touch base with you to ensure you’re completely comfortable.
Step 4:
Discuss Your Treatment Plan
Dr. Julovich will share his observations with you and show you the X-rays from earlier. He’ll go over any issues he might see as well as which treatments could be used to address them. Before you leave, he will work with you to put together an ongoing care plan that is suited for both your goals and budget.
Dental Patient Forms
Have you ever been frustrated juggling clip boards and filling out forms in waiting rooms? Does the time it takes to fill out these forms seem like it’s longer than the time you spend actually visiting with the dentist? At Trinity Dental, we set aside ample time to work with you one on one no matter what, but by filling out forms from the comfort of home, we’re able to meet with you faster. Plus, no need to shuffle through papers, insurance cards, and other documents in our waiting room!
Dental Insurance
We know that patients invest a significant amount of money in dental insurance every year. It’s important to us that we help you make the most of that investment. That’s why we work closely as in-network providers with many PPO, including Delta Dental, Cigna and Anthem Blue Cross. Additionally, we’re happy to process and file claims with any insurance provider to maximize your coverage.

Online Payment & Financing
We ask that patients pay for any part of treatment not covered by insurance at the time of service. For your convenience, we accept payment in cash, local checks, and major credit cards. You can even pay on-line using the Quick Pay system. For patients who are unable to finance the full cost of treatment out-of-pocket, we partner with CareCredit, a 3rd party healthcare financier.
